You can obtain the Soul-Shriven Style by completing Cadwell`s Silver. You will receive the Soul-Shriven Style per in-game mail once you completed Cadwell`s Silver.

Is it part of the ESO Base game or do I need a DLC or ESO plus subscription?

To be able to obtain the Soul-Shriven Style you don't need any DLCs or Chapters.

What do I need to craft the Soul-Shriven Style?

To craft the Soul-Shriven Style you need the style material Azure Plasm which are also obtained by defeating enemies found at Dark Fissures and Dark Anchors.
Once you complete Cadwell's Silver you will get some Azure Plasm also send per mail.
